Gangs of New York - US Army Enlistment Form

A prop enlistment form used in the 2002 movie "The Gangs of New York" starring: Leonardo DiCaprio, Daniel Day-Lewis & Cameron Diaz.

These paper prop US Army enlistment forms can clearly be seen in the scene where Amsterdam takes one from the recruiter as shown in the pictures, and also again on the shipping docks greeting the newly arrived immigrants. This volunteer enlistment form is A4 in size and has been printed on both sides.
